Information about other people

The Refer a Client form asks for information about someone other than the person filling it in. Please share only what is needed for us to follow up, and only where you have the authority or permission to share it. Do not use this form to send detailed medical records or clinical documentation.

We use what you send to respond to your enquiry, review referrals and discuss availability, consider job applications, and keep in contact with you about the request you made. We do not use it for anything else.
